In 1976 Raymond Williams commented, ‘Culture is one of the two or three most complicated words in the English language.’ Such implied difficulty has not prevented Bloomsbury Academic, since the 2000s, from publishing around forty series of their well-produced and generously illustrated Cultural Histories, with, according to their website, a further fifty in progress. Each series contains six volumes, each book covering, in theory, the same chronological period (antiquity, the Middle Ages, the Renaissance, the Enlightenment, the age of empire and (...) the modern age), though there is some variation depending on precise topic. The idea is that one can use these books not only to read ‘horizontally’ about a subject across time, but also ‘vertically’ through different subjects in the same period – a idea made easier by the e-texts of the series on Bloomsbury's website. Jansen and Wall suggest a new way of defending hard paternalism in clinical research. They argue that non-therapeutic research exposing people to more than minimal risk should be banned on egalitarian grounds: in preventing poor decision-makers from making bad decisions, we will promote equality of welfare. We argue that their proposal is flawed for four reasons.First, the idea of poor decision-makers is much more problematic than Jansen and Wall allow. Second, pace Jansen and Wall, it may be practicable for regulators (...) to uncover the values that a potential research participant holds when agreeing to enter a research project, so their claim that we must ban such research projects for all if we are to ban them for poor decision-makers looks to be unmotivated. Third, there seem to be cases where the liberty to enter the sort of research project Jansen and Wall discuss is morally weighty, and arguably should outweigh concerns of egalitarian distribution. Fourth, banning certain types of research, which seem on the face of it to offer an unfavourable risk-benefit ratio, would have unwelcome consequences for all clinical research, which Jansen and Wall do not recognize. Individuals who undergo transplantation must chronically be maintained on an immunosuppression regimen for rejection prophylaxis to help ensure graft survival. Current rejection prophylaxis consists of using a combination of calcineurin inhibitors, mTOR inhibitors, (...) antimetabolite agents, and/or corticosteroids. These agents have collectively improved the short-term outcomes of renal transplantation, but improvements in late/chronic graft loss and recipient survival have lagged significantly behind challenging the field of transplantation to develop novel prophylactic agents. There have been several clinical trials conducted within the last 5 years in an attempt to bring such novel agents to the commercial market. These trials have resulted in the US Food and Drug Administration approval of extended-release tacrolimus, as well as belatacept, which has the potential to replace calcineurin inhibitors for rejection prophylaxis. Other trials have focused on the development of novel calcineurin inhibitors, costimulation blockade, kinase inhibitors, and inhibitors of leukocyte migration. While these later agents have not been FDA-approved for use in transplantation, they remain noteworthy, as these agents explore pathways not previously targeted for allograft-rejection prophylaxis. The purpose of this review was to consolidate available clinical trial data with regard to the recent developments in rejection prophylaxis in kidney transplantation. In the care of patients with disorders of consciousness (DOC), some ethical difficulties stem from the challenges of accurate diagnosis and the uncertainty of prognosis. Current neuroimaging research on these disorders could eventually improve the accuracy of diagnoses and prognoses and therefore change the context of end-of-life decision making. However, the perspective of healthcare professionals on these disorders remains poorly understood and may constitute an obstacle to the integration of research. We conducted a qualitative study involving healthcare professionals from an (...) acute care university medical center. A short questionnaire captured demographic data as well as the experience of participants with DOC patients. A semi-structured interview was used to explore attitudes toward ethical issues identified in a previous literature review. Qualitative content analysis of interviews was conducted with the NVivo software. Accurate diagnosis among DOC is often regarded as a challenge, but this was generally not the case for our participants because most reported high confidence in DOC diagnoses. However, participants reported struggling with prognosis, especially because of its essential role for end-of-life decision making and communication with families. Variability of opinion between healthcare professionals was reported and identified by some as a minor issue while others stressed how families struggle with different medical opinions. End-of-life decision making encompassed a large proportion of ethical challenges in these patients, and the removal of artificial nutrition and hydration created significant discomfort in a minority of participants. The concept of futility was subject to wide-ranging understandings with both favorable and unfavorable opinions. Our data suggest that to ensure the incorporation of new evidence-based advances, attention should be directed to the real-world practices and challenges of accurate diagnosis and prognosis. Given pervasive challenges in end-of-life care, we recommend improved training of healthcare professionals in the care of patients with DOC, particularly in end-of life care, understanding the context of decision making, and determining how to optimally integrate new neuroscience research on the care of patients with DOC. We suggest that the phenomenon of uncertainty monitoring in nonhuman animals contributes richly to the conception of nonhuman animals' self-monitoring. We propose that uncertainty may play a role in the emergence of new forms of behavior that are adaptive. We recommend that Smith et al. determine the extent to which the uncertain response transfers immediately to other test paradigms.

In an attempt to learn from COVID-19, this essay features six responses to the question: what did COVID-19 teach us, expose in us, or purge out of us when it comes to spiritual formation in Christ? Each response was written independently of the others by one of the coauthors. Diane J. Chandler focuses in on how COVID-19 exposed grievous inequities for ethnic groups in the American church and broader society. Kelly M. Kapic reminds us of the goodness of human finitude (...) and how COVID restrictions have forced many of us to embrace our limitations. Siang-Yang Tan reflects on eight lessons he has learned during this pandemic year in his role shepherding a local church. James C. Wilhoit calls us to consider the structures that are needed for local church leadership to make wise and godly decisions in times of crisis. Richard Peace draws our attention to what might be learned from the forced monasticism brought about by COVID-19 quarantines. Finally, Ruth Haley Barton pauses to consider the interdependence of human life that has been dramatically illustrated by this pandemic. While these six responses certainly do not exhaust all there is for the church to learn from COVID, we present them in the spirit of “O Lord, teach us what we do not see” and hope they will inspire your own reflections. In the preface to this work, Thayer explains that his purpose is to present "the classic writings of pragmatism" defined as "the original and formative expressions of this philosophy articulated by its most eminent spokesmen." The selections are from Peirce, James, and Dewey as well as brief readings from Mead and C. I. Lewis. Each selection is accompanied by a brief introduction. In addition to these selectional introductions, there is also a two-part general introduction. The first part is a (...) short historical piece by Thayer situating pragmatism in the context of western thought. The second part is a 1931 article by Dewey outlining the development of pragmatism in Peirce and James. These informative introductions should be of value to the philosophical novice who finds himself in many anthologies afloat on the sea of primary sources without the aid of the historical context necessary to fix his position. The individual selections have been carefully chosen and well coordinated. Insofar as they cluster about the central theme of the epistemological nature of pragmatism, they avoid the pitfall of attempting to elucidate the entire philosophy of many thinkers in one volume. The Peirce section includes in addition to the usual articles, "The Fixation of Belief" and "How to Make Our Ideas Clear," selections from Peirce's later writings including the 1905 article, "What Pragmatism Is" in which he differentiates his pragmatism from that of James. The section on James incorporates important sections from The Principles of Psychology together with the famous essay "The Will to Believe" and two lectures on the pragmatic concept of meaning and truth from Pragmatism. The Dewey selection provides a judicious collection of his writings from the 1896 article on the "Reflex Arc Concept" advocating the integration of organism and action to the general analysis of intelligent action in his 1938 book, Logic: The Theory of Inquiry. Also included is a chapter from The Quest for Certainty describing the application of pragmatic method to the realm of moral value. The contents of this volume are rounded out with a piece by Mead on social consciousness and the social self and an article by C. I. Lewis on the pragmatic conception of the a priori. Thayer has produced an excellent anthology of pragmatic philosophy well suited to any course in classical American philosophy emphasizing an epistemological approach.--W. J. L. (shrink)

Objective: To develop an approach for seeking informed consent to examine tissues retained from a previous study of sudden infant death syndrome as part of a study on asthma, and to document responses and participation rate.Design: Pilot open-ended approach to 10 volunteer SIDS parents, followed by staged approach to seek consent from the target SIDS families for the asthma study.Participants: Parents of SIDS infants known to SIDS and Kids Victoria and parents of SIDS infants from the 1991–2 SIDS in Victoria (...) case–control study.Main outcomes: Qualitative responses of the piloted parents and study parents, and participation rates.Results: The pilot group responses were used to refine the written material to be provided. Of the 72 families for which contact details were available, 45 gave verbal consent for contact by the Victorian Institute of Forensic Medicine regarding the asthma study, three refused and 24 did not respond to two letters. Thirty-three completed consent forms, all positive for participation in the asthma study, giving a positive response rate of 73% .Conclusions: The use of postmortem tissue for research is acceptable to the next of kin when an approach is sensitive to their concerns and needs and is made by experienced counsellors from a familiar organisation. Despite the painful memories evoked by the approach of the research group, the acceptance rate among those who could be contacted was high. (shrink)

J. L. Austin, in "Ifs and Cans," proclaimed the common hope that we soon "may see the birth, through the joint labors of philosophers, grammarians, and numerous other students of language, of a true and comprehensive science of language." The problem has always been with the "joint labors" part. Philosophers have always been willing to issue linguists dictums and linguists have been happy to teach philosophers "plain facts." Austin’s general view of language, and his particular notion of performative utterance, can (...) be found in the writing of J. R. Firth, the most commanding British linguist of Austin’s generation, but Austin never refers to Firth. In the present volume, however, we find clear and exciting evidence of genuinely joint labors on the part of philosophers and linguists. They stem from a summer conference in 1969 rounded out with contributions from notables. To two thick issues of Synthese the editors have added a dazzling piece by Saul Kripke, two substantial pieces by James McCawley and J. R. Ross, a short paper by Paul Ziff, and a reprint of P. F. Strawson’s "Grammar and Philosophy." This is an invaluable book and the best book among the many now available concerning the interaction of linguistics and philosophy: worth the cost, which the contributors attempted to reduce through foregoing royalties. The philosophers in this volume hold, or hold intriguing, the view that the semantics of a natural language can and must, in effect, be a theory of truth for a language in much the manner that Tarski suggested, and provided, for artificial language: the recursive specification of biconditionals in which the left hand gives the structural description of an object language sentence and the right hand, the truth conditions in the metalanguage. In "homophonic" translation this requirement can be trivially satisfied simply by mentioning the sentence on the left that one uses on the right: one makes the requirement non-trivial by forcing enough into the recursive specification so that one captures the native speaker’s implicit semantic competence. In this volume, the "orthodox" Davidsonian program, which takes the syntax of the metalanguage to be standard predicate logic, is ably argued by John Wallace ; Richard Montague, David Lewis, and Jaakko Hintikka would want an intensional logic covering modality and propositional attitudes. The linguists who find this philosophical climate most appealing are called "generative semanticists": McCawley, Ross, George Lakoff and others argue that any proposed semantic rule will eventually prove necessary to syntax too and that, hence, the deepest level of syntactical form will be equivalent to semantic form. Whatever the ultimate fate of this joint program, it leads here to much exciting interaction between linguists and philosophers: linguists who welcome the machinery and conceptual standards of modern logic, and philosophers who try to grasp the specifics of crucial issues in recent linguistic theory. Even if Quine’s doubts, here sketched, and Chomsky’s currently unpublished more technical objections should be well-founded, nonetheless the joint labor will have been very much worthwhile. Aside from this general debate about semantics, there are several papers covering more specific issues. The papers of J. A. Fodor, Terence Parsons, and Ross concern adverbs and the logical form of action sentences; several papers, particularly B. H. Partee’s, examine "Opacity, Coreference, and Pronouns." In all these papers one notes the fulfillment of Austin’s hope that philosophic and linguistic arguments should become intermixed, if not at times properly indistinguishable. Perhaps the most enjoyable and exciting paper stands aside from linguistics: Saul Kripke’s "Naming and Necessity." Kripke here argues quite informally for the separation of analytic, a priori, and necessary that is required for a Kripke style, S5, modal logic with de re modalities. "Gold is a yellow metal," for example, turns out to be contingent, while "Heat is the motion of particles" is necessary but a posteriori ; and that philosopher’s stone of stones, "The morning star is the evening star," is discovered to be necessary but a posteriori.—J. Here find four chapters as the first publication of the Woodbridge Lectures and Kant Lectures ; chapters 5 and 6, "On the Biological Basis of Language Capacities" and "Language and Unconscious Knowledge," have, substantively, appeared as parts of other books. This is an excellent book for philosophers, almost wholly given to theoretical-philosophical issues. A complementary process to the one William James outlined respecting the reception of a new idea is that the purveyor of the new idea, stung by dismissive (...) and misleading rejection and acceptance, eventually ascribes his opponent’s characterization of his own position to them. This is Chomsky’s attack on his opponent’s anti-scientific, anti-realist, speciesist, methodological, and mystical dualism. And Descartes is hardly championed. Scientific linguistics and psychology study the species-specific, componentialized, innately and biologically structured, largely unconscious and introspectively inaccessible, cognitive organs. H. Putnam’s new Kantianism is pummeled, as is Dummett’s anti-realism. The logical behaviorist-ordinary language critics are reproved, and it is curious how quaint the Wittgensteinian style impossibility-of-psychology-and-there-is-nothing-to-explain razzmatazz now seems. This is a compact, sharp, and coherent expression of themes explicit or implicit in Chomsky’s earliest work, and one not likely to be wholly pleasant to those joyed by what he was saying in the late 1960s. Whatever one’s persuasion, it also suggests that Chomsky has tried to respond to his myriad philosophical opponents comprehensively, in depth, and in detail—virtues which many of them seem shockingly unwilling to extend to him., never having read any other work of Chomsky’s or other linguists. On being shown that his paper was straight irrelevant to more than a decade of active work in linguistic theory, he seemed to take it that this suggested the moral and intellectual bankruptcy of Noam Chomsky and linguistic science.)—J.L.
